There are two options when filing a claim against health insurance –

• Direct Billing - Network hospital/ clinic

If you get treatment at a hospital or clinic listed on the insurance provider's network, you don't have to file a claim. Show your medical insurance card at reception, and the hospital or clinic will note down your insurance details. They will then directly contact the insurance provider for payment. You don't have to make any payment or claim. 

• Claim Reimbursement - Off-network hospital/ clinic

 If you get treatment at a hospital or clinic not listed on your insurance provider's network, you will have to make a little effort in filing a claim. You will have to pay out of pocket for the treatment but keep all the receipts. Submit the receipts with photocopies of your medical reports to your insurance provider. If your employer insures you, you can submit it to the relevant department at your workplace. They will send it to the insurance provider. After review, if your procedure and treatment were covered under the medical insurance plan, the insurance provider will reimburse you for your expenses.
